Before your dog's first visit to Gus' Playground, please complete the following forms. These forms help us provide the best possible care for your dog.
- Contract: This agreement outlines the terms and conditions of boarding your dog at Gus' Playground LLC.
- Health Policy Acknowledgment: This form confirms that you understand and agree to our health policies, which are in place to protect all dogs in our care.
- Vaccination Waiver: Medical exemptions accepted with waiver & vet documentation, such as a titer test. Additional precautions may apply.

- Download and Print: Download the forms, print them out, complete them by hand, and bring them with you on your dog's first day.
