To ensure the safety and well-being of all our furry guests at Gus' Playground LLC, we require pet owners to complete specific forms before bringing their dog for boarding. These forms help us understand your dog's health history, vaccination status, and your preferences for their care during their stay.
Accessing and Completing the Forms
To make the process as convenient as possible, Gus' Playground LLC offers two options for accessing and completing the required forms:
Online Submission: Fill out and submit the forms directly through our website for a quick and easy process.
Download and Print: Download the forms, print them out, complete them by hand, and bring them with you on your dog's first day.
Required Forms Overview
Contract: This agreement outlines the terms and conditions of boarding your dog at Gus' Playground LLC.
Health Policy Acknowledgment: This form confirms that you understand and agree to our health policies, which are in place to protect all dogs in our care.
Vaccination Waiver: Exemptions accepted with waiver & vet documentation, such as a titer test. Additional precautions may apply.
